Patient Safety Incident Response Framework (PSIRF) Maturity Assessment

Organisations are at different stages in their PSIRF journey, and while national guidance sets clear expectations, many teams have asked for more practical support.

That is why we created the PSIRF Maturity Assessment Tool — a practical framework designed to help organisations measure progress, share learning and embed continuous improvement.

Co-produced with NHS organisations, Acute and Mental Health Trusts, Community Interest Companies and an award-winning provider of PSIRF training, the tool reflects real-world experience and supports a shared vision of excellence.

Maturity Assessment Tool

The full Maturity Assessment Tool reviews 35 key lines of enquiry (KLOEs) across 9 domains, with core KLOEs identified for smaller organisations.

If you would like to discuss how we can support you to assess your organisations level of PSIRF maturity, and support continuous improvement and development, please complete the expression of interest contact form or scan the QR code.

PSIRF Events

We are excited to invite you to our PSIRF Maturity Assessment Tool: Pilot Evaluation Shared Learning event!

This is a fantastic opportunity to gain exclusive insights from the independent evaluation of the ASW Assurance PSIRF Maturity Assessment Pilot conducted across seven organisations in the South West.

This event will cover some of the content from the in-person event in Exeter (April 2025) for those that were unable to join us, and will further develop the evidence for Patient Safety excellence as we showcase insights and outcomes from the independent evaluation (commissioned through Health Innovation South West).

ASW PSIRF Maturity Assessment Outcome Report (example)

Please see the attached example report that was created in line with the outputs received by participating organisations; this report is not specific to any organisation and is to be used as an example of the report style and content you may receive following a review.

Local Benchmarking Report

Following completion of the pilot programme we compiled a bench marking report to enable all participating organisations to understand their PSIRF maturity against their regional peers. This has been anonymised and is available to view.

We will continue to offer benchmarking across peer organisations where applicable as participation progresses.

Notable Practice Guide

Following the conclusion of the pilot programme, we developed a notable practice guide with the consent of participating organisations that demonstrated exemplary maturity through their evidence submission for key lines of enquiry within their assessment.

The Internal Audit Network Benchmarking Report, August 2025

ASW Assurance is a member of The Internal Assurance Network (TIAN).

TIAN is a collaborative group of NHS internal audit and counter fraud providers across England and Wales.  It aims to promote the sharing of best practices, resources, and insights to support the quality and effectiveness of client services.

We used the results from participating pilot organisations to answer a series of predefined questions as part of a national benchmarking exercise, conducted through our involvement with TIAN.

The benchmarking exercise involved 59 organisations, comprising of four Integrated Care Boards (ICBs) and 55 provider Trusts from across England. Participating TIAN members included 360 Assurance, MIAA, Audit Yorkshire, Audit One, Barts Assurance, and ASW Assurance. The benchmarking outcome report, which provides a high-level summary of the data analysis, accompanying commentary, and examples of identified good practice.
